APPOINTMENTS...

...are accepted by phone.  We are open from 9 to 5, Monday through
Thursday and 9 to noon on Friday. Unfortunately we are not open to
new annual appointments at this time.

THE PROCESS...

  STEP 1.  Unless you will be paying cash for the visit at the time of
service, verify that the office is contracted with your insurer. The
practice accepts most private insurances, but the insurance market
is complicated and contracts can change  with little notice. It is the
patient's ultimate responsibility to pay for services received,
so
patients are asked to verify coverage through their own insurers.
  
  
STEP 2.  Download and complete this bundle of forms. We must
ask this of all patients who are seeing the doctor for the first time in
this office -- even if they have seen her elsewhere.

  STEP 3.
 Use the included Records Release Form to have your records sent to the office. We must have your medical records prior
to your appointment.

  
STEP 4.  Arrive with all forms completed or come early. It can take
up to 30 minutes to complete all forms and if paperwork is not
completed, your appointment may have to be rescheduled.

  
STEP 5.  Present your insurance card at check-in.

  
STEP 6.  Keep your appointment! You should receive a reminder
call from our office. We need at least 24 hours notice for any
canceled appointments. Please know that we charge a $35 fee for
those who those who miss appointments without sufficient notice.
